Family must live within Santa Rosa City limits and show proof of receiving at least one of the following: Neighborhood Services Youth Development Program membership, Social Security Supplemental or Disability Income, public assistance through Sonoma County Human Services Department or City of Santa Rosa Housing Authority, or Medi-Cal Benefits.
