Charles L. Shor Epilepsy Scholarship

Applicants must have physician-documented, active epilepsy with seizures occurring within the past 12 months and/or currently be taking epilepsy medication. Applicants should not be candidates for surgery. A letter from the applicants’ physician or treating healthcare provider is required with documentation of ongoing treatment for epilepsy; (Download the Physician Diagnosis Verification Form); be a graduating high school senior (including GED) or current college student.
